Doctors notes, written after each meeting with you, have a long tradition as a key communication tool between your doctor and other health care providers. Now the following types of notes, with some exceptions, must be made available to you, too, securely, “without delay,” and at no charge:
- consultation notes
- discharge summary notes
- history & physical
- imaging narratives
- laboratory report narratives
- pathology report narratives
- procedure notes
- progress notes
This change took place on April 5, 2021 through the final provision of the bipartisan federal 21st Century Cures Act which became law in 2016.
Often referred to as “Open Notes,” the central goal is to improve communication between you and your clinicians. Such openness is intended to increase your knowledge of your own health, clarify instructions (including medication regimens), promote compliance, and help ensure accuracy, all contributing to improving the quality of your care .
